  • Position Status: Full-time, 40 hours per week
  • Schedule: Rotation – 10:00 am – 6:00 pm, 11:00 am – 7:00 pm, work 3 weekend days per month *with on call responsibilities

Responsibilities

Here’s what to expect as a Licensed Practical Nurse (LPN), Residential Services:

  • Assess and support the clinical needs of individuals and provide consultation regarding changing signs and symptoms of illness.

  • Administer medications, including prescribed treatments, injections, and other health-related interventions; monitor and document participants' vital signs and health status.

  • Monitor individuals for signs of illness or changes in condition and initiate appropriate treatment or interventions.

  • Maintain accurate, up-to-date health records for each individual, including medication administration, treatments, and health assessments; ensure documentation complies with state regulations (6400, 6500, 6100) and agency policies.

  • Communicate with physicians, specialists, and family members regarding changes in individuals’ health and required treatments and interventions.

  • Participate in discharge procedures, interdisciplinary meetings, and provide input on Individual Support Plans (ISPs) to ensure healthcare outcomes are implemented and monitored.

  • Conduct quarterly audits of Medication Administration Records and perform routine audits of health-related documentation to ensure regulatory compliance.

Qualifications

Minimum requirements as a Licensed Practical Nurse (LPN), Residential Services:

  • Must possess a current and valid nursing license (LPN) in Pennsylvania
  • Must have at least two (2) years of work experience in the medical field and ideally work experience with individuals diagnosed with intellectual and developmental disabilities
  • Must have clinical competencies in all aspects of nursing and be able to apply nursing skills and sound judgment during the nursing process
  • A valid driver’s license and 6 months driving experience
  • Access to a vehicle with valid insurance, registration, and inspection

Preferred, but not required:

  • Knowledge specific to the needs of individuals with intellectual and developmental disabilities, autism, and/or the aging population
