Here’s what to expect as a Licensed Practical Nurse (LPN), Residential Services:
Assess and support the clinical needs of individuals and provide consultation regarding changing signs and symptoms of illness.
Administer medications, including prescribed treatments, injections, and other health-related interventions; monitor and document participants' vital signs and health status.
Monitor individuals for signs of illness or changes in condition and initiate appropriate treatment or interventions.
Maintain accurate, up-to-date health records for each individual, including medication administration, treatments, and health assessments; ensure documentation complies with state regulations (6400, 6500, 6100) and agency policies.
Communicate with physicians, specialists, and family members regarding changes in individuals’ health and required treatments and interventions.
Participate in discharge procedures, interdisciplinary meetings, and provide input on Individual Support Plans (ISPs) to ensure healthcare outcomes are implemented and monitored.
Conduct quarterly audits of Medication Administration Records and perform routine audits of health-related documentation to ensure regulatory compliance.
